Wellbeing Transition Volunteer

What is the role

You will liaise with staff to identify community groups, services and any other support available for patients to transition onto at the end of their 12-week programme within Wellbeing Services. A significant element of the role will involve researching and liaising with external community groups, as well as supporting patients for a period of time during their initial transition to these services. 

Role tasks
  • Liaising with staff and patients to identify interests and sessions for patients to transition onto, following their time on the Wellbeing programme
  • Researching into community groups within the local area, making use of existing databases and resources
  • Liaising with key people from community groups to identify sessions for patients to attend
  • Supporting patients with their transition from the Wellbeing programme, and attending initial sessions with patient
  • Working with staff, reporting on community group availability and identifying opportunities for further collaborative working
