Would you like the reassurance of specialised palliative care within your own home? Whether you need a nursing visit or just a friendly phone call, day or night, then the St Michael's Hospice at Home team can help.
The Hospice at Home service takes the specialist skills of St Michael’s Hospice into an individual’s home, enabling people to have a choice over their place of care. The team provides specialist palliative care when there are problems needing more expert input such as complex symptom control, emotional and psychological support. The team can provide hands-on nursing care in the short term, whilst a package of care is being set up.
Referrals to Hospice at Home
Referrals are made by Macmillan Nurses, Hospital Consultants, Hospital Doctors, Nurses and all allied health professionals including nursing and care homes. The service draws on resources from St Michael’s Hospice and works in close partnership with our colleagues in primary care. All care is co-ordinated with your GP or District Nurse. We involve you and your family or friends in all decisions.
